Budzi się on po dziewięciu latach śpiączki, będącej efektem odniesionych ran, w tym stratą lewej ręki. Niedługo po odzyskaniu przytomności, szpital w którym przebywa Snake zostaje zaatakowany przez uzbrojonych napastników i bohater musi ratować się ucieczką. Pomaga mu w tym tajemniczy pacjent z zasłoniętą twarzą, przedstawiający się jako Ishmael.
